If you are coming to a Brethren Retreat sponsored event (i.e. Summer Camp, Fall Retreat, Snow Camp, Winter Retreat) and you have medication that you take you need to have one of these forms filled out (front and back) by your parents. Medications need to be brought in their original bottles and need to be given to the designated medical person at registration.
