Financial Aid Application - It is the desire that no family is denied an opportunity to attend a Camp Shipshewana session due to financial constraints. A Campership may be requested from the Brethren Retreat scholarship fund by submitting a Financial Assistance Form.
Medication Form - If a camper has medications (prescription or over the counter) they must be checked in with the Camp Nurse fo rthe safety of all campers. Medications must be in their original containers. A Medication Form needs to be completed and signed by the Parent/Guardian.
Paintball Liability/Permission Form - During our Camp Shipshewana summer programs we offer paintball as an extra activity. In order to participate in paintball a wavier of liability & permission form signed by the camper's Parent/Guardian must be on file. A new form must be signed for each different Camp Shipshewana program
Activity Fee: $20 per camp session (includes staff supervision, equipment rental, and 200 painballs)
Equine Activity Liability/Permission Form - We also offer pony rides during some of our summer camp sessions. This is a lead pony ride in a corral area. In order for a camper to participate in any equine activities a wavier of liability & permission form signed by the camper's Parent/Guardian must be on file. A new form must be signed for each different Camp Shipshewana program. Cost: $10 per camp session (includes staff supervision, helmet, pony care, and 6 rounds in the corral)
